  • Address for communication including door number, street/locality, city, district/state and pin code.
  • Phone number - Mobile number and Landline number.

  • Date of birth
  • Place of birth

  • Martial status - single or married.
  • Date of marriage which is an optional field.
  • Mother's maiden name
